Ten percent of the net earnings from Making Tired Eyes
Smile® will be shared with others who are dedicating themselves to creative
service to others. Once a year a portion of these proceeds will be awarded as
grant(s) to those who have been serving in this way for at least a two year
period on the date of submission. Submissions are accepted October through
November each year. The grant(s) will be awarded on March 1st.
The first submission period will be October through
November 2007.
Please submit the following:
- Name,
address, phone number, e-mail and if applicable, web site
- A
description on your creative expression in serving others
- The date
activity began
- Visual
documentation of the service (i.e., photos, examples)
- Two
letters of recommendation from people served or from caregivers / responsible
parties of the ones being served
- The details
for the use of funds, including the budget
- Your vision and mission
statements
